As a consumer of IMPACT services, you have the following rights:
- Consumers will have the right to be accorded human respect and dignity on an individual basis in a consistently humane fashion.
- The right to be protected from harm from any form of abuse neglect or mistreatment.
- The right to privacy and confidentiality relative to their treatment
- The right to access of all the consumers’ personal records and information
- The right to manage personal funds.
- The right to access funds when the provider is payee.
- The right to complaint and grievance procedures.
- The right to be informed of the financial cost of services.
- The right to be informed of the need for parental or guardian consent.
- The right to a written statement of services to be provided.
- The right to give written consent prior to be being involved in research.
- The right to be fully informed of all services provided
- The right to give informed consent
- The right to refuse services as the law permits
- The right to be free of discrimination or prejudice
- The right to receive treatment regardless of your financial status.
- The right to individualized treatment.
- The right to be involved in the assessment of your Treatment.
- The right to treatment in the least restrictive environment.
- The right to refuse treatment whenever you wish without reprisal.
- The right to manage personal funds.
- To not be denied, suspended, or terminated from services or have services reduced from exercising any of your rights.
- To confidentiality of HIV antibody and/or AIDS status
- To know the name of the person coordinating your care.
- To have the opportunity to evaluate the agency’s service.
- You have the right to file a grievance up to the level of the Executive Director.
- You have the right to informed consent.
- You have a right to go to advocacy/self help groups
- You have a right to refuse services from the person you get assigned and a right to request a specific clinician, if available.
- You have a right to not complete releases of information and a right to revoke them, if already completed.
- You have a right to an investigation and resolution of any alleged infringement of rights within specific time frames. You will not face retribution or retaliation if you act in good faith in making a report.
- If you are a minor above the age of 14, you are entitled to active participation in treatment without parent or guardian knowledge or consent.
- The legal guardian of a minor, except where a minor is above the age of 14, and chooses not to involve parents in treatment will be given a copy of these rights and grievance policy.
- The right to have access to courts and attorneys
- The rights to enforce rights through courts or appropriate administrative proceedings.
- The right to be informed of commitment status.
- The right to be informed of the means for accessing advocates, an ombudsman, or rights protection service.
- The right to be free from seclusion, restraint, drugs, or other interventions.
- Emergency determinations limiting privacy shall be reviewed and documented frequently.
- Written and informed consent must be signed by the personal representative less than 14 years old before photographs are taken.

These rights are available to you as a consumer of mental health services. Some limitations may occur from time to time based on individual circumstances.
